Health Requirements for School Entry

You must provide the school with a proof that your child has met the following health requirements. These requirement apply to both public and private schools, preschools or child care centers in Hawaii.

1- A tuberculosis clearance test within one year prior to Preschool and K-12 school entry.

2- A physical examination within one year prior to school entry date.

3- The complete series of required immunizations.

Your health care provider will document the required health information on the official Student’s Health Record (Form 14). This form becomes part of your child’s official school records and is transferred along with other official records when your child changes schools.
