You must provide the school with a proof that your child has met the following health requirements. These requirement apply to both public and private schools, preschools or child care centers in Hawaii.
1- A tuberculosis clearance test within one year prior to Preschool and K-12 school entry.
2- A physical examination within one year prior to school entry date.
3- The complete series of required immunizations.
Your health care provider will document the required health information on the official Students Health Record (Form 14). This form becomes part of your childs official school records and is transferred along with other official records when your child changes schools.
The health requirements for tuberculosis screening, immunization and physical examination must be completed by the first day of school. If your child does not have proof of a physical or has not met all immunization requirements, a doctors statement (with the date/time of next visit) or appointments slip from your childs doctor or clinic will enable your child to enter school provisionally. Your child must follow through with these appointments and must meet all the requirements within three months of provisional entrance.

School Year: Two semesters, approximately 176 days of classroom instruction.
School Begins: First week of September.
School Ends: Prior to June 11
Recess: Winter- two weeks including Christmas and New Year's Day. Spring- last full week in March.
School Choice: Public school students must attend the school in the area where they live, unless granted a "geographic exemption". For GE information, call the school where attendance is desired.
Kindergarten Eligibility: Children who are five by Dec. 31 of the school year. Nor mandatory.
Registration Requirements: Children registering in Hawaii for the first time need:
1- Birth Certificate
2- Certificate of release and proficiency from the last school attended.
